  • Grands capitules violet-mauve de 3 à 8 cm, à bractées épineuses
  • Lieux incultes de l’Europe méridionale, des régions méditerranéennes, de l’Afrique septentrionale, de l’Asie de l’Ouest

  • Selon la croyance populaire, les marques blanches des feuilles proviendraient du lait de la Vierge allaitant l'enfant Jésus

    • Les effets sont plus marqués pour 140mg de silymarine 3 fois par jour, soit 420 mg/jour (équivaut à 21 grammes de drogue si la silymarine y est présente à 2 %)

Propriétés

Propriétés de la plante

  • Hépatoprotecteur par les flavonolignanes [1], [2], [3], [4], [5], activités liées à diverses fonctions de récepteurs et transporteurs situés dans les membranes cellulaires, modulation du TNF-alpha et des sélectines, propriétés antioxydantes et inhibition de la voie de la lipoxygénase, inhibition de l'oxyde nitrique synthase inducible iNOS, du facteur nucléaire kappa B et réduction de la synthèse du collagène, effets médiés par l'ADN et l'ARN [6]
  • La silymarine agit par deux mécanismes :
    • C'est un stabilisateur de membrane (elle inhibe les systèmes de transport membranaires des hépatocytes, rendant plus difficile l’absorption des toxiques, inhibe la peroxydation des lipides [7] et augmente la captation des radicaux libres produits par les substances hépatotoxiques) ; elle contribue au maintien du pool glutathion responsable de la détoxification hépatique (enzymes de phase 2), réduit l’augmentation des transaminases et des phosphatases alcalines, protège vis-à-vis du stress oxydatif [8]
    • Elle stimule l’activité de la polymérase A, donc augmente la synthèse des acides nucléiques ribosomaux et le nombre de ribosomes dans les hépatocytes, ce qui accroît la biosynthèse enzymatique, la synthèse protéique et stimule la capacité de régénération hépatique
  • Prévention et traitement de l’hépatotoxicité du tétrachlorure de carbone [9], de la galactosamine [10] et de la phalloïdine [11], [12], [13]
  • Antidote de l'intoxication par l'amanite phalloïde [14], une méta-analyse portant sur 452 patients atteints d'intoxication par l’amanite phalloïde montrent une différence significative dans la mortalité en faveur de la silibinine (mortalité 9,8% contre 18,3% avec traitement standard, p <0,01) [15], la silibinine prévient l’absorption des amatoxines de l'amanite phalloïde par les hépatocytes, protégeant ainsi le tissu hépatique, elle stimule les ARN polymérases ADN-dépendantes, augmentant la synthèse d’ARN [16], [17], [18]
  • Amélioration de la durée de vie dans la cirrhose [19]
  • Antivirale vis-à-vis de l'hépatite C [20], [21], inhibe la polymérase à ARN du virus [22]
  • Prévention du carcinome hépato-cellulaire par une synergie Cynara scolymus, Silybum marianum, Cochlospermum angolense [23]
  • Néphro-protecteur [24]
  • La plante est de plus galactogène [25], [26], [27] et anti-dépressive
  • Propriétés anti-cancer de la silymarine [28], propriétés anti-inflammatoires et activité anti-métastatique par modulation de protéines spécifiques [29]
    • La silymarine exerce une activité anti-cancer par l'intermédiaire de multiples mécanismes : initiation de la carcinogenèse, promotion et progression. Son effet anti-métastatique est notable. De nombreuses études cliniques montrent une réduction de la toxicité des agents chimiothérapiques en association avec la silymarine [30]
    • La silymarine inhibe la prolifération des cellules tumorales de divers cancers :
  • Diminue la toxicité rénale du cis-platine [42]
  • La silymarine (flavonolignane) présente des effets immunosuppresseurs [43], [44], [45]
  • La combinaison avec de la phosphatidyl-choline augmenterait l'absorption et l'efficacité de la silymarine [46]
  • Effet hypoglycémiant [47]
  • Activité cicatrisante (plaies d'épisiotomie), et antalgique [48]
  • L'huile extraite des graines, sous-produit de la fabrication de la silymarine, possède aussi des propriétés anti-oxydantes [49]

Indications

Indications de la plante entière (phytothérapie)

  • Hépatites et hépatopathies latentes, stéatoses, cirrhoses
    • Dans les hépatites virales B ou C, la silymarine agit favorablement sur l’élévation des transaminases et des gamma-GT, mais pas sur la charge virale [50]
  • Prophylaxie et traitement des maladies hépatiques engendrées par des toxiques (prévention des chimiothérapies, protection de la cellule hépatique lors des chimiothérapies anticancéreuses) [51]
  • Protection contre les effets toxiques de la phalloïdine avec de meilleurs effets si l’administration est prophylactique (donc si les flavonolignanes sont absorbés avant la phalloïdine ou tout autre toxique)
  • Cancer :
    • Chimioprotection
    • Le traitement à la silibinine peut cibler l'inhibition des voies prolifératives de divers cancers par des mécanismes moléculaires tels que les récepteurs à la tyrosine kinase, les récepteurs aux androgènes, les STAT (signal transducer and activator of transcription), le NF-kappaB, les voies régulatrices du cycle cellulaire et les voies de signalisation de l’apoptose [52]
    • La silibinine aurait des propriétés antinéoplasiques prometteuses et potentialiserait l’effet des anticancéreux (carcinomes de la peau, du sein, du poumon, du colon, de la vessie, de la prostate et du rein) [53]
    • Certaines régressions sont spectaculaires (par exemple d'un hépatocarcinome) [54]
  • Anticancer, anti-Alzheimer, anti-Parkinson, antidiabétique [55]
  • Dépression nerveuse, stimulation de la lactation
  • Les infusions et extraits alcooliques en délivrent pas suffisamment de silymarine pour obtenir un effet anti-hépatotoxique marqué (effet insuffisant de la TM)
    • En effet, la dose usuelle est de 12-15 g de drogue, et les effets les plus marqués ont été le fait de 140 mg de silymarine 3 fois par jour, soit 420 mg/jour (si elle est présente à 2 % dans cet échantillon, il faudrait 21 g de drogue). Néanmoins on peut supposer que l'extrait total aura une activité amplifiée par rapport aux flavanolignanes seuls
  • Utilité dans les surcharges hépatiques en fer et les hémochromatoses [56]
  • Atoxique (même à 20 g/kg chez la souris)
    • En Belgique et en Suisse, la silibinine (Légalon Sil®) est utilisée dans l’intoxication par l’amanite phalloïde sur des données expérimentales (même s’il n’existe pas de véritable antidote de l’intoxication phalloïdienne). La silibinine agirait en limitant l’absorption intestinale ainsi que le transport intra-hépatocytaire des amatoxines et en stimulant l'ARN polymérase. Une étude rétrospective de cas publiés aurait relevé une association statistique entre l’utilisation de silibinine et une réduction de la mortalité (comme avec la N-acétylcystéine). Posologie : 5 mg/kg en glucosé 5% IV en 2 heures, à répéter toutes les 6 heures pendant 6 jours. Source : Centre anti-poison de Belgique http://www.poisoncentre.be/article.php?id_article=33
    • La silibinine serait également active sur la réplication du virus de l'hépatite C [57]
  • Potentialités dans les maladies auto-immunes [43]
  • Le pic plasmatique de silymarine est atteint 2 à 6 heures après une prise orale et sa demi-vie d’élimination est d’environ 6 heures [58], [59]

Effets indésirables éventuels et précautions d'emploi

  • Pas de toxicité aigue ou chronique de la silymarine (même à 20 g/kg chez la souris)
  • Possibilité d'allergies aux plantes de la famille des Asteraceae
  • Interactions pharmacocinétiques :
    • Surveiller d'éventuelles interactions médicamenteuses, la silymarine diminue l'activité des enzymes du cytochrome P-450 (CYP), de l'enzyme UDP-glucuronosyltransférase (UGT) et réduit le transport de la glycoprotéine P (P-gp) [60], [61]
    • Risque d'interactions avec la warfarine (CYP2C9) [62]
    • La silymarine réduit en particulier l'activité de l'enzyme CYP3A4 et l'activité de l'uridine diphosphoglucuronosyl transférase (UGT1A6/9), elle peut donc altérer le métabolisme hépatique de certains médicaments, leur pharmacocinétique et ainsi entraîner une toxicité accrue [63], mais d'autres travaux montrent que l'extrait de chardon-Marie n'a pas d'influence significative sur les activités des CYP1A2, CYP2C9, CYP2D6 ou CYP3A4/5 [64]
    • Éviter l'association avec des traitements chimiothérapiques surtout métabolisés par le CYP3A4, il est conseillé de faire des fenêtres thérapeutiques
  • Interactions possibles avec :
    • Certains antidiabétiques antihyperglycémiants [65]
    • Inhibiteurs de protéase (indinavir, saquinavir) [66]
    • Augmentation de l’INR chez les patients sous warfarine
    • Diminution de la clairance du sirolimus chez les patients transplantés rénaux insuffisants hépatiques
